Here are all the forms you may need to help manage your Wright-Patt Credit Union® accounts including check re-order, direct deposit, HSA and more.
WPCU® has partnered with a new IRA document provider giving you a more user-friendly, self-service option for submitting transaction requests for your IRA online.
To help you navigate our new IRA Service Center, we’ve developed the following step-by-step guide.
Once you submit an IRA transaction request through our new IRA Service Center, our team will review your request and either complete your request or contact you if they have any questions. Once our team has completed your IRA request, you will receive a confirmation email.

Please note: In addition to requesting these transactions through our trusted third-party’s IRA Service Center, you can also visit your nearest member center and a partner-employee will be happy to assist you.
IRA Service Center:
Utilize our new IRA Service Center to access forms and more. Here you can start the process for:
- Opening an IRA
- Start the process of opening a new WPCU Roth or Traditional IRA. Once you’ve completed the opening steps, a partner-employee will follow-up with you to complete the account opening process.
- Making a Transfer, Distribution, Rollover or Withdrawal Request
- Move your Traditional IRA assets or Roth IRA assets from another financial organization to your Traditional or Roth IRA at WPCU.
- Request a direct movement of assets from an employer-sponsored retirement plan to your WPCU IRA.
- Request or set up a distribution from your IRA.
- Withdraw funds from your IRA.
The first time you log in, you will be asked to:
- Authenticate your login by entering a verification code sent to the email address associated with your WPCU account
- Please note: Not sure what email address is assigned to your WPCU account? Log into Mobile or Online Banking and select “My Contact Information” to see the email address you’ve selected for your WPCU account.
- Click “Accept” to authorize the app to access your WPCU IRA
- Confirm your identity to match to your WPCU IRA by providing your last name, birthdate and Social Security number.

Please note: Beneficiary changes cannot be made through our IRA Service Center. To add and/or modify IRA beneficiary information select the form below.
Managing Beneficiaries:
- IRA Designation of Beneficiary Form – Complete this form if you wish to change the beneficiaries on your Traditional or Roth IRA.
- Once you complete the form, please bring it to a nearby member center or mail it to: Wright-Patt Credit Union, Attn: Deposit Services/IRA, 3560 Pentagon Blvd., Beavercreek, OH 45431.

If you prefer to request transactions on your IRA using traditional paper forms, that option is available below. Select the IRA form you need from the list below. Once you complete the form, please bring it to a nearby member center or mail it to: Wright-Patt Credit Union, Attn: Deposit Services, 3560 Pentagon Blvd., Beavercreek, OH 45431.
- Traditional IRA Custodial Application – Complete this form to start the process of opening a Traditional IRA at WPCU.
- Roth IRA Custodial Application – Complete this form to start the process of opening a WPCU ROTH IRA.
- IRA Deposit Form – Complete this form to make a deposit to your IRA at WPCU.
- Traditional IRA Distribution Form – Complete this form to request a normal or early distribution, charitable distribution or transfer to another IRA.
- Roth IRA Distribution Form – Complete this form to request a qualified distribution, early distribution or transfer to another ROTH IRA.
- Traditional IRA Transfer Request Form – Complete this form to move your Traditional IRA assets from another financial organization to your Traditional IRA at WPCU.
- Roth IRA Transfer Request Form – Complete this form to move your ROTH IRA assets from another financial organization to your ROTH IRA at WPCU.
- Roth IRA Direct Conversion Request Form – Complete this form to convert your Traditional IRA to a Roth IRA.
- IRA Direct Rollover Request Form – Complete this form to request a direct movement of assets from an employer-sponsored retirement plan to your IRA at WPCU.

Once you submit an HSA request through our HSA Service Center, our team will review your request and complete your request or contact you with any questions. Once our team has completed your HSA form request, you will receive a confirmation email.

Utilize our new HSA Service Center to access forms and more. Here you can:
- Request a Rollover or Transfer
- If you’d like to move your HSA assets from another financial organization to your WPCU HSA.
- If you currently have a Medical Savings Account (MSA) or Health Reimbursement Account (HRA) with another custodian and want to roll those funds over to your WPCU HSA.
- If you have received funds from a distributing Health Savings Account (HSA), Archer Medical Savings Account (MSA), or Health Reimbursement Account (HRA) and want to roll those funds over to your WPCU HSA.
- Make a Contribution
- If you would like to make a contribution to your WPCU HSA via a check, another WPCU account, or from an account at another financial institution.
- Add a Distribution or Recurring Distribution
- If you want to request or set up a distribution (or recurring distribution). This movement is known as a "qualified funding distribution.”
- Access Tax Forms
- If you would like to access a copy of your 1099-SA and/or 5498-SA tax forms (available at a future date).

Select from the list below if you'd like to make any of the following updates to your WPCU HSA:
- HSA Application – This form is an application and membership agreement used when opening a WPCU Enhanced HSA.
- Adjustment Request – This form should be completed if you have made a mistaken contribution or mistaken distribution and need a correction.
- Beneficiary Form – Complete this form if you wish to change the beneficiaries on your HSA.
- HSA Authorized Signer – Complete this form if you wish to add an authorized signer to your HSA.
- Revocation of HSA Authorized Signer – Complete this form if you wish to remove an authorized signer from your HSA.
- HSA Transfer – Complete this form if you would like to move funds from an HSA at another financial institution to WPCU.
